UCSF Departments of Pathology and Laboratory Medicine Training Verifications
A training verification is an official confirmation of a physician’s completed education and/or training. It is a standard step for medical staff appointments, employment, licensure, and residency/fellowship credentialing.
Processing Fees & Payment
Due to the high volume of requests for training verifications, UCSF’s Departments of Pathology & Lab Medicine require an administrative processing fee of $50 for each official verification that needs to be filled out and/or signed for all trainees who have been out of the program for over 6 months. General email requests that do not need official documents filled out or verification for trainees who have been out of the program less than 6 months have no fees. Verification services are rendered only after we receive a confirmation of payment.

Who to Contact on Completing your Training Verification Request
After payment, please email the appropriate program administrator below with your training verification request/form to be completed and the trainee’s signed release form. If you require more than one training verification (e.g. residency and fellowship), please include all appropriate program administrators in your initial email request.
