- Billing information is included on the Surgical Pathology Consultation Requisition Form. If billing patient’s insurance, please provide front and back of patient’s insurance card. HMO and other insurances require prior authorization – clinician’s office is responsible for verifying and obtaining. Include a copy of the authorization printout when sending.
- We bill the international institutions directly as we do with other clients. Please provide institution name, address, phone number and email address so that we can email the invoice when ready.
