Principle Creatine phosphate and ADP react with serum CK to form ATP which is coupled to the hexokinase-GPD reaction, generating NADPH. The rate of increase in absorbance at 340 nm is proportional to CK activity.
Interpretation CK rises within 6-12 hours following an MI and peaks in 24 hours. CK isoenzymes of cord blood, combined with quantitation of the BB bands, give an excellent prediction of subsequent neurological deficits in children with asphyxia neonatorum.1

1. Major increases: severe myocarditis, progressive muscular dystrophy2, polymyositis and dermatomyositis, alcoholic myopathy, post-seizures, muscle injury secondary to trauma, crush, or gunshot wounds; also increased after exercise and after surgery. Increased during last 2 weeks of pregnancy and post-partum.

Note: Elevated CK in amniotic fluid indicates fetal death3.

2. Mild increases: acute psychosis4, IM injections, electrical cardiac defibrillation, patients in remission with malignant hyperthermia.

3. Normal: Angina pectoris, pericarditis, pulmonary infarction.
