Collection Instructions The blood drawer must sign the Blood Bank Requisition, in the "collected by" space.

Special instructions The patient's name and MRN on the requisition and specimen must match exactly. Inadequately labeled specimens will be discarded - no exceptions. A crossmatch cannot be added to this test. If the antibody screen is positive, an antibody workup will automatically be initiated. If a clinically significant alloantibody is identified, an antibody titration will be performed. If the patient appears to be Rh negative, a DU (weak D) test will automatically be performed.
