Principle Colorimetric measurement of p-nitrophenol produced by enzymatic hydrolysis of p nitrophenyl phosphate.


                        Alkaline Phosphatase
p-nitrophenyl phosphate         ->              p-nitrophenol + H3PO4 + H2O

Interpretation 1. Increased in: increased deposition of calcium in bone; osteitis fibrosa cystica, Paget's disease, healing fractures, osteoblastic bone tumors, osteogenesis imperfecta, osteomalacia, ricketts, polyostotic fibrous dysplasia, late pregnancy, childhood, and Vitamin D deficiency. Liver disease with obstruction of biliary tree nodules (tumor, cyst, abscess, TB); biliary duct obstruction by stone or cancer; cholangiolar obstruction in hepatitis; drug induced cholestasis (e.g. phenothiazines). Hyperthyroidism, I.V. injection of albumin, post-MI (congestive heart failure). Hemolysis.

2. Decreased in: excessive vitamin D, milk alkali syndrome, scurvy, pernicious anemia, celiac disease, malnutrition, hypophosphatasemia.

Special instructions EDTA, citrate and oxalate inhibit alkaline phosphatase activity. Moderately/grossly hemolyzed serum will not be assayed.

Stability Alkaline phosphatase activity may increase 5-30% at room temperature. Stable 2-3 days at 2-8°C; one month at - 20°C.
