Principle Lactate dehydrogenase catalyzes the conversion of lactate and NAD to pyruvate and NADH. The rate of NADH production is proportional to LD activity.
Interpretation LD occurs in all organs, but elevation occurs in the serum only after organ damage.

1. Greatest increases (2-40x) occur with megaloblastic anemia, extensive carcinomatosis, and with severe shock, rhabdomyoloysis, and anoxia.

2. Moderate increases (2-4x) occur with MI, but also pulmonary infarction, granulocytic anemia, hemolytic anemia, mononucleosis, muscular dystrophy. Following MI (24 hr), increased LD occurs and may persist 10-14 days.

3. Mild increases: hepatitis, delirium tremens (skeletal muscle origin), and muscular dystrophy. Isoenzyme electrophoresis can determine the possible organ source of LD.

4. Decrease: Could be due to storage, certain drugs, or inhibitors.

Synonyms LD;
Stability Serum stable 2-3 days at 2-8°C. Do not freeze. Certain LDH isoenzymes (LD-4 and LD-5) are more sensitive to cold. Specimens may have variable decreases in LD activity when stored at 2-8°C, depending on their isoenzyme composition.
